The Sarbanes-Oxley Act passed by Congress in 2002 requires that employees must be given the means to anonymously notify federal regulators or an audit committee of potential wrongdoings within their companies. The Executive Council at Mariners Church serves as the audit committee. This form has been generated to allow employees to alert the Executive Council regarding any potential wrongdoing they observe or become aware. The form will automatically be forwarded to the Executive Council and the employee/sender of the message will remain anonymous to all parties. The employee may identify themselves if they so choose.
